creating mold for blow molding,

My company has developed a new product to be used in the recreational fishing industry here in the USA. We are seeking a company with blow mould plastic capabilities and mould creation and production. I am seeking a company that can produce a Semi hard plastic product in florescent Orange color (My best guess is that a low density polyethylene would be a good choice. The colorant will likely need to have UV (ultraviolet light) stabilizer for use outdoors).  We have a computer three dimensional design to scale of the product. Basic measurments are 5.25 x 6.0 x 1.50 inches, approximate weight of plastic less than 2 ounces. We are seeking a company with the capabilities to obtain a lead product which will be six and one half inches long, one inch wide and nine thirty-seconds thick, semi rounded corners with a one eighth hole one half inch from one end to attach 75 feet of nylon cord. Approximately three thirty-seconds in diameter with approximately fifty pound strength. I am attaching jpeg files of the plastic design as well as 2 dimensional designs with dimensions and a third jpeg file of a current product on the market here in the USA to give you and idea of what the final product would look like.
