Custom Metal Golf Ball Marker

We are a US company and need to establish a relationship with a manufacturer to be our ongoing supplier of custom metal ball markers. (Standalone markers - no clip or other attachment or holder) We would like to get the following information from you as a first step in the process:

(1) Capabilities from the standpoint of marker custom sizes, metal options and surface printing options,

(2) The process and requirements for submitting artwork and proofing

(3) Capabilities to direct ship to us in the US,

(4) An example of your pricing for metal golf ball markers, to provide us an indication of the approximate cost. For the purpose of providing this to us assume a metal ball marker, 1 1/2 inches in diameter with two color printing. Separately, let us know what the cost add-on is for stamping the back of the ball marker with company information.
